Primary Care Clinic

Visiting a doctor may seem odd when you’re healthy and feeling normal, but it is actually the best time to maintain your health and be aware of any potential risks or issues earlier. A primary care provider offers an extensive range of preventive wellness and screening and healthcare services tailored to your individual needs.

At CareMed, we have an outstanding team for primary care clinic that focuses on Preventive Care, Regular Annual Wellness Visits, Chronic Care Management, and Transition Care Management. We help patients with an effective range of primary care services, along with expert medical professionals.

Why Primary Care Matters

The effects of primary care have shown longitudinal continuity of care for the whole patient. Having access to primary care finds treatment before the development of more severe problems. People who receive primary care have fewer preventable emergency department visits and hospital admissions.

Primary care is considered to improve the quality of life and health care outcomes with improved immunization, blood pressure control, dental health, and reduced mortality. It promotes prevention; proactively supports patients suffering from chronic illnesses, and engage patients in self-management and decision-making.

Preventive Care

We believe prevention is the first step in building a healthy life, so we work hard to achieve care goals of prevention for every one of our patients. It consists of any medical service that defends against health emergencies, including doctor visits.

We focus on reducing preventable diseases by offering exceptional preventive care services. The goal of preventive care is to assist people in maintaining their health by eliminating diseases at the start before they become catastrophic. It also plays a crucial role in keeping health care costs low.

Regular Annual Wellness Visits

To fulfill constant care goals, at CareMed, we encourage regular annual wellness visits. In addition to visiting a physician when an illness or injury occurs, regular annual wellness visits enable primary care providers to monitor your diagnostic readings such as heart rate, blood pressure, etc. and prescribe medication or treatment to bring these readings up to healthy levels.

This type of primary care is particularly crucial for middle-aged and elderly patients to create or update a personalized prevention plan. This plan helps prevent illnesses based on your current health and risk factors. It keeps track of chronic diseases and infections that might occur in the body, such as cancer, diabetes, and heart disease, to help patients get cured in early stages.

Chronic Care Management

Chronic Care Management is recognized as a crucial component of primary care that contributes to better health and care for individuals. If you are a patient with multiple chronic medical problems such as arthritis, asthma, diabetes, depression, or another qualifying condition expected to last at least 12 months or until the death of the patient, Chronic Care Management services may be for you.

We make sure care measures are met and provide quality chronic care services. Our team at CareMed will be in continuous contact every month and ensure to refill all medication timely. There will be 24/7 access to a healthcare professional in case of any urgency, along with assistance with setting and meeting your health goals.

Transition Care Management

At CareMed, we take full responsibility for the care of patients following discharge from a hospital setting and returning to home or an assisted living facility. The goal is to alleviate the percentage of avoidable patient readmissions after discharge by offering more effective care at the transition.

We are committed to follow and fulfill all care gaps. Our transition care team contacts patients within 48 hours of discharge from a hospital or a short-term rehab facility. Our program enables timely and consistent patient discharge information flow, follow-up care visits, and patient outreach with dedicated provider services to meet continuous medical-care goals.
